so far they have given me canesten HC cream, fluconazole 150mg, aqueous cream B.P and double base gel. canesten HC cream made me worse and nothing else is working and now it looks as if the doublebase gell is making it worse any moisture makes it worse and they just keep giving me creams and it makes it worse, ive had this problem over 3 months now

For one thing if your wearing tight underwear the cream could be thinning the skin on the testicles causing inflamation and if you shave the area, dont.  Having red testicles with no burning or itching could just be nomal skin color and bending over checking them constantly could be forcing more blood there causing the red appearance.
Irritation on the inner thighs can be triggered by sweat as you mention. Once you get the irritation under control with the creams, try wearing loose fitting boxer shorts so the area can get cooled. Also try using natural soaps and fabic softerners as they also can cause skin irritations.
