Mouth/Oral Care Tracker (Possibly for things like TMJ)

While using this site, i have tried desperately to find something that covers TMJ for some type of tracker, or just plain old pain in the mouth. It would be wonderful, because lately i have been tracking alot of my mouth issues with my migraine tracker. It gets confusing trying to figure out which is which. I just started using the pain tracker, but some of the options are missing certain categories/entry issues, and its kind of a hassle trying to figure which side of your face is hurting when placing the locator on the figure. If it has to be set up that way, maybe make an image of just a persons head down to the lower part of the neck.  Anyways, it was just a thought.

Something else i thought of is maybe adding a time zone of what time you take something for the pain. and what you take. that way you can keep track of the time you take something, and keep a record of if the meds work or not.

Ok, here are some things that i came up with for right now:
Events/Symptoms:
Anxiety
Appetite
Agitation
Anger
Fatigue
Headache- how bad
Grind or clench teeth?
Joint Pain
Muscle Pain
Nausea
Pain
Relaxed
Stress/ Emotional stress
Change in Weather
Open Mouth? How wide?
What kind of food did you eat? Crunchy/ Chewy? thick or large foods?
ENT Discomfort
Cracking in the Jaw?
tenderness in the face, jaw joint area, neck and shoulders, and in or around the ear when you chew, speak, or open your mouth wide
Jaw get stuck or lock?
Difficulty chewing or a sudden uncomfortable bite – as if the upper and lower teeth are not fitting together properly
Swelling on the side of the face
neckaches, dizziness, earaches, hearing problems, upper shoulder pain, and ringing
Treatments:
Anti-anxiety meds
antidepressants
Exercise
Therapy
Anti Inflammatory
Heat
Cold
Injection
Massage
Pain Killers
NSAIDS
OTC Meds
Rest
Mouth Guard
Transcutaneous electrical nerve stimulation
Trigger-point injections
Uncomfortable bite
Trouble Sleeping?
Map/Outline:
Make a map region of the Head/face and make it clickable to choice a region of pain that is present.
Make the map of the head/face show the ears, and go down to the neck region.
Make a map of the inside of the mouth to show the upper and lower part of the jaw so that people can click on a region of where a tooth/teeth are in pain.
Make a timeline and severity of pain during the day/night time hours.
Make a note When to see a doctor
Seek medical attention if you have persistent pain or tenderness in your TMJ or if you can't open or close your jaw completely.
Make an entry spot of when last visit to dent/doctor, or upcoming visits. Include an entry place to put contact info for the specialists.


This is all i can think of for the moment, if i can think of anything else, i will try to add it. Otherwise maybe you can come up with some things that i missed.

Thanks for the suggestions!  We will certainly consider adding a dental/oral tracker.  Great idea!

What are some must-haves for you if we could do this tracker?  What do you think should be included?
