High WBC

Hello everyone. I went to my doctor about a month ago for stress (daily vomiting, sometimes multiple times, unintentional weight loss [10% of my body weight], and general uneasy feelings). She sent me for an abdominal ultrasound (normal) and bloodwork. My blood work came back with the following results and she's sending me for more.

24 year old, female, with children
Platelets appear adequate. Red cells appear normal.
Neutrophils: 6.46 (ref range:1.8-7.0)
Lymphocytes: 1.62 (ref range:1.0-3.2)
Monocytes: 0.81 (ref range:0.0-0.8)
Eosinophils: 1.21 (ref range:0.0-0.4)
Basophils: 0.00 (ref range:0.0-0.2)
All are units x 10*9/L

I am still vomiting, still feel unwell, still losing weight, and have now added sunken eyes and easily bruising to my list. A nurse friend suggested I make another appointment to discuss leukaemia, and I have, but it's 3 weeks from now. I was wondering if anyone had any input here. Thank you!

As a possibility, take a look at the last comment from swinkie:

which also mentions a high D-dimer, which you probably also had during the HELLP if you were over-coagulating.

Probably, but maybe not. E.g., there is a test where they put a wire with sensors down a persons throat for a day, which records episodes of acid getting into the esophagus. Patients sometimes say they felt nothing, but had acid reflux episodes nevertheless.

Some causes of reflux: infection in the stomach valve, obesity, innate structural defect in the valve.

I don't know that HELLP microdamage to the liver would show up on a sono or not. Smoking shouldn't alter a sono, AFAIK.

Show the dermographia to any doc you visit.

Many people with EoE are negative on allergy tests, yet improve during food elimination trials. You can ask and verify that on an active EoE group.

So you should at least be aware of MCAS, also called MCAD. Think of Mast Cells as the master cells that direct other immune cells. They can behave in various ways in various people and cause a large variety of problems.

You can find "niacin flush" videos on youtube. Most people flush head and neck only; you probably would flush much, much more and might get frightened. You very probably have overactive Mast Cells. MCs can in turn stir up eosinophils.

Most leukemias are in lymphocytes. Eosinophilic leukemias are very rare. Likewise, the vast majority of people with eosinophilia do not have cancer. The Eos instead are multiplying for non-cancer reasons, like parasites or allergy.

(As you have seen, I'm giving a lot of signposts by briefly referring to things like MCAS and MCs -- which requires some researching.

The more you know, the less of the limited time you will waste during a doc visit with basic questions. It must be a lot to absorb now...

Ultrasound showed nothing on my liver, gallbladder, spleen or kidneys. Although I do need to mention to my doctor that I had forgot to fast completely, and that those normal results were actually after smoking.

Wouldn't I feel acid reflux in my chest?

But that being unlikely is really good... And that makes me feel better at least. Thank you.

Our labs send them off at the end of the day... that's why I need to wait to get my paper once my doctor has received results. However, they will call me as soon as they get them and I will be allowed to go pick them up then.

No trouble swallowing, although in the evening I find the left side at the top of my throat aches which makes both eating and vomiting that much less fun.

My doctor knows about that, she had sent me for allergy tests. I've had them since, but she said it was just stress.

I don't believe so. Doesn't sound familiar.

HELLP might have damaged your liver, which possibly results in lessened production therein of "coagulation proteins" - a lack of which can result in easy bruising. (Also look for bleeding after brushing your teeth.)

But that doesn't explain the eosinophilia, etc.

Acid reflux can cause eosinophilia, so then vomiting likely can, too. But what's causing the vomiting, if it's not from pure stress/nerves? I'd keep a food log.

As you can see, things are not simple - so you'd want to rule out the most dangerous causes and then settle in for a possibly long and frustrating period of diagnosis.

But having Eosinophilic Leukemia would be very unlikely.

A CBC is done with an automated counter: a machine which also prints results right then. At least you can hopefully ask someone if your platelets are okay. I stress this because you might be very worried and can hopefully not have to wait weeks for a doctor appointment - if that's possible in Canada.

I just looked up HELLP, which often includes headaches.

If your platelets are low, together with past HELLP, then maybe you qualify for an immediate doc visit because of possible DIC. But without anemia, that tends against MAHA and DIC, I'd guess.


For most of human existence, eosinophils had the main job of fighting tiny parasitic worms in the gut such as helminths. They also can be involved in food allergies/sensitivities.

Do you ever have trouble swallowing?

Esosinophilic GastroIntestinal Disorders (EGID) are a group of emerging diseases, and not every doc is fully aware of them. The usual kind of skin prick allergy testing very often tells nothing about EGIDs, it's useless.

That's important, I think. It perhaps tell something about rampant Mast Cells, am immune problem. You are probably correct that it seemed random but might have had an unknown trigger. However, there are random attacks of flushing in MCAD/MCAS, a Mast Cell disorder. Can you draw a line on your sternum with your fingernail (dermographia)?

Do you have bad reactions to bee stings?

Have you ever taken niacin?

I don't have a family history of autoimmune disorders. I have a couple family members who have various types of cancer (breast, liver), and heart disease. That's it.

I got a copy printed at my doctor's. I'm able to get a copy when they get the results only (Canadian thing maybe?).

I just got an allergy test done in February because I had a "random" attack of hives with a rash. I was clear. I've also never have any food sensitivities.

The only real health problem I've ever had was severe pre-eclampsia which turned into HELLP. After I had my son (Jan 2012), I had to be readmitted to the hospital for my blood pressure (was 212/?? when I was readmitted). During that week I had an MRI and CT scan. They found that my blood vessel in my neck/base of my head were restricted and that I had nasal polyps. Nothing was said again after that. And other than increasing headaches, I haven't had a problem since.

If you get a CBC, make sure to get a copy for yourself, That can often be printed for you instantly. If they won't instantly give you a copy, then never go to that place again.

For other tests, you should also get copies - though they cannot probably be given instantly.

Then the next thing to consider would be a food allergy or sensitivity. The 1st suspect would probably be dairy. There are about 5 usual food suspects. Sometimes the problem is a component of foods such as histamine or salicylates.

As far as bruising, a blood cancer creates easy bruising/bleeding when the cancer cells get into the marrow and crowd out the production of the good cells such as platelets. If you ask a doc on the telephone for and get an immediate CBC to verify that your platelet number is still okay, that should set your mind at ease without waiting 3 weeks. Then you can begin thinking of other causes. E.g., with a damaged intestinal lining, you might not be absorbing vitamin K from foods. Or an allergy can affect blood vessels, sush as when a person gets dark circles under their eyes.
