By chance is the side of your face that is swollen the side on which you slept?
I don't get visible swelling. However, if I sleep on one side I wake up with many symptoms on that side, my guess being that it is all inflammation related.
For example, if I sleep on my left side, I'm likely to wait up with strong tingling on the left side of my face, and more aches/bizarre sensations all through the left-hand side of my body. Same deal if I sleep on my right, I'll wake up with more symptoms on that side.
As you may have guessed, I try to sleep on my back as I do best that way.

Were you tested for co-infections, meaning other diseases carried by the same tick that carries Lyme? Not only does Lyme have different symptoms in different people at different times, but the coinfections confuse the picture with their own set of symptoms interacting with Lyme. And coinfections don't always respond to the same antibiotics as Lyme.
It's also possible that 3 weeks of antibiotics didn't kick the Lyme infection, tho 'mainstream' MDs can be stubborn in that regard. Even without antibiotics, the rash usually goes away by itself.
I'd get back to the MD right away and see what s/he says. Is your MD a Lyme specialist? If not, then I would consider finding one, sometimes called an LLMD for shorthand -- it's not a formal title, just one that means the MD has a more open mind about these things than mainstream docs.
