Severe Scrotal pain, constipation and rectal pain

Hello - I saw my GP about a month ago after experiencing burning after urination.  He did a urine test suspecting UTI and put me on macrobid(antibiotic) while he waited for the results.  A week passed and there was no relief. Went back to GP and he said the results of the urinalysis and urine culture were normal but I told him about that the pain had since intensified.  He did chest X-ray, abdominal X-ray, Tran rectal ultrasound, Digital Rectal Exam, PSA test suspecting it was prostatitis and while he waited for the results, he put me on a 10 day dose of Cipro.  The results were normal for all tests with the exception of the testicular ultrasound and abdominal X-ray which showed moderate constipation.  I used a laxative to clear the blockage.  The scrotal ultrasound showed small bilateral hydrocele's in both testicles.  My symptoms at this point would only get worse, the pain near the rectum and scrotral area was unbearable.  After about the 7th day of being put on Cipro, went to the ER with scrotal swelling.  the ER did a urinalysis and put on another 10 day of Cipro.  After being on about the 15 day of Cipro, symptoms started to lessen.  Few days ago, the symptoms came back with a vengeance.  I feel the same way I had felt a month ago. Went back to ER, the did a testicular ultrasound which showed the hydrocele, the urinalysis and urine culture were both normal. The ER referred me to a urologist.  Saw the Urologist yesterday who said I likely had some sort of infection and it's probably gone.  The sensitivity and pain may take months to dissipate due to the sensitive nature of the anatomy and he put me on a daily dose of Celebrex.  Last night, I couldn't sleep well... the pain was unbearable, at 3am which briefly helped.  I have had numbness, achey scrotum and rectum all day.  In fact, I have noticed that the pain is always in the recturm and scrotum area.  Is there any insight anyone can share? I had a colonoscopy 4 years ago and it was all normal.  Could it be a problem related to the colon? I am going through a lot of anxiety and lot of pain. Any help would be greatly appreciated.

By the way, I am a 39 year old healthy male.  At times I drank heavily but I don't eat red meat and I don't smoke.
