What is going on down there



29 year old male reproductive question…The perineum area is not flat, the whole area behind the scrotum is hard and round - like one large lump - several inches long and wide. Not sure if this additional information could be relevant - the penis angles very sharply to the left, one of the testicles seems to be lost in the sack. And, there is also a problem getting or keeping an erection. The male was a runner for many years and currently cycles quite a bit.

Thanks for that history, well written.  So, definitely suggest you see a doctor for evaluation.  Could you have a mass in your scrotum? https://www.mayoclinic.org/diseases-conditions/scrotal-masses/symptoms-causes/syc-20352604.  With what you describe, it sounds like a definite possibility.
