30 yr old male, sexually active. Around jan 17 i had frequent and constant urge to urinate. Jan 19 went to ER, they treated me for Gon and chlamydia. Frequency never stopped. Went to dr got tested for gon chlamydia and came back neg. Went to urologist and tested me for chlamydia, ureaplasma, Mycoplasma, gonorrhea. All neg.  Cultured urine 3 times no infection found. During this time urethral discomfort, and occasional small clear discharge. Had ultrasound on bladder, kidney, doc said everything seemed fine. Also prostate exam, doc said seemed fine. Went to an std clinc, told story, they gave me antibiotics for trich. Symptoms still persisted. I went and got herpes type specific blood test which came back neg for hsv1, equivocal for hsv2. Retested 2 weeks later, neg hsv2 .20, neg for hsv1 .25. By this time, symptoms felt much better, only irritated went i ejaculate. Went and got a second opinion from different urologist, put me on doxycycline for 2 weeks, symptoms better, but still after i ejaculate seems to irriate urethra a litte, got a second course of doxy, and i almost feel 100% but i masterbated once this week, felt ok. Waited 2 days, did it again, and have a little irritation again. Please help me. Its been 3and half months. This irritation seems to linger, i dont know what it is, i feel my sexual history is low risk. Really messing with me mentally.

Well seen a new urologist. He spent about 2 maybe 3 minutes with me. Diagnosed me with prostatitis without even doing any tests. Im thinking about traveling out of state to see a specialist next.

Hi thanks for your response. I drink plenty of water. Ive had about 4 urine cultures along with 2 urine tests for chylamidia and gon. I also take plenty of ibuprofen, nearly everyday for the past 3 months. Its just very tough because every time after sex it gets irritated and takes a day or so to get back to normal, hurting my marriage. I wish i could just be diagosed and treated

Have they done a urinalysis?  Sounds like you might have a uti or something. Glad you didn't do the cystoscopy, not worth the pain, to find nothing.  Other tests are more conclusive before doing anything invasive like a cysto.  I wonder if you are dehydrated?  Drink plenty of water and cranberry juice in case its a uti, also may try Ibuprofrin in case you have an inflammation.  Other than that, be positive and listen to doctor, don't over think it, it will drive you crazy.  best Wishes.

Ok so i went to the urologist. But the day before i went i had sex. A few hrs later i noticed a red spot with 2 darks marks on it. I was freaking out. I went to the urologist and told him my previous symptoms and showed him the red mark. He said it wasnt any kind of std. I told him ive been feeling much better but still once in awhile i get irritation after sex or just urination. He said he did not want to preform a cystoscopy. And being that nothing has been found in my tests that we should just leave it alone for a little while and see what happens. I was still alarmed about the red spot so i had a hsv blood test done which came back negative. So now i dont know what to do. I am feeling better but i know these symptoms are not in my head. Im thinking of visiting another urologist or indfectious disease doctor in another month or so. I got out of the shower today and noticed a drop of clear discharge from my penis. It didnt have a smell. Jst small clear and stretchy. I dont know whats going on. Ive had so many tests and everything seems normal.

I have gone through the complete thread. I do not know much about this subject. Many a time bladder and prostate problem linger on in spite of treatment. In holistic therapy Kati snan is often recommended. Search Kati Snan( Kati is waist snan is bath.) Google kati snan or waist bath. It is suggested in many groin diseases. There is no harm in doing a tub bath. study and try it. Best of luck.

i think you need to let the doctor see it with a camera. I dont think you will find out anything in your cyst. It seems that anything that s going on its very high up on the urethra near your meatus. Cystoscopy is being done carefully, it is aseptic and a protective lube is being used. You wont be injured. Dont stop until a doctor helps you get over this. Aftermath of an infection maybe but maybe it hasnt gone 100%. We ll see. Keep me updated

Thanks again for your time. Ive entertained the idea of prostatitis or CPPS. Which seems possible. Injury or infection seems to fit my symptoms. This cystoscopy makes me nervous if it is injury because i dont want it to get even more injured. Is it possible if i had an infection, that its gone now and i could be just dealing with the aftermath of it

Well im on my 2nd urologist. My 1st one did a sensitivity culture. He insisted nothing was wrong w me and told me i could seek a second opinion. I was on different drugs at the time and dont know if it could have messed up the tests. He never mention anything about that.
Its hard to say if the ejaculation irritated my urethra during treatment because i tried to not masterbate. Probably only 2 maybe 3 times. When i did my urethra still felt sensitive. I would only get that ticke/itch feeling briefly and not last long at all.
During the first 2 months the irritation was obvious. The tickle amd itch feeling throughout the day. Overtime that started to pass and be less obvious, only when ejaculation took place it would flare up and would need a few days to settle. Sometimes a strong urination would aggravate it as well. I look inside my urethra all the time to see if i can see anything and it seems normal. When i squeeze or milk the urethra, that seems to be what makes it aggravated. I also seem to dribble a lot. Again if i chill out for a few days it seems ok, but then if i ejaculate once or twice its back. Its towards the top probably about a half inch down.
Although after the doxy the discharge has been nearly non existent.

I m really sorry you re going through this. I know its very frustrating and I am pretty certain that there will be a solution to this very soon. May I ask something. Do you have the irritation after ejaculation when you are on antibiotics? I think you just havent had the right antibiotic yet. I ve seen cases like your one having symptoms for years and then they suddenly bumb onto a good microbiologist that finds what organism is causing this. If they manage to culture this, with a proper antigram they will prescribe the best antibiotic for you. Have you ever considered going to a different doctor?

So i had another culture done thursday. I have been feeling better. Friday i masterbated and after my urethra feel irritated again. Throughout the day i had just a few feelings of a tickle or itch at the tip of my urethra area. Then this morning after i urinated i had a small amount of clear sticky substance mixed in with my urine. I just want to cry,  I just want to be better. Ill be talking to the doctor monday.

I think two weeks will be just fine unless you ve been taking them for a very long time. As far as the cystoscopy thing its  something that your doctor will decide if its worth it. I believe it wont show anything. Try to abstain from sex or masturbation for two to three weeks. i know its a long time but sometimes it helps not to put pressure on a wound so it will heal properly. Imagine a wound that is being teased all the time would open and open again. In the middle of wound healing connective tissue is formed before it starts to remodel its self into normal skin or mucosa. Connective tissue is hard and non elastic so its easier to reopen a wound on that stage of wound healing. As far as the microorganisms  that i said cannot be scrapped off it means that they belong to the natural flora or the vagina and should be there no matter what. It doesnt mean that they are pathogens. Only when out or control or in sensitive individuals. Try to get to the bottom of this. Let me know of any news

Thanks again for your response. Im gonna stay off antibiotics for 2 weeks and see what happens. I fell fine today, this whole thing has been weird. Do u think i should get a urethral swab in 2 weeks?  also u think i should go ahead with the cystoscopy then? Also when u say antibiotics can't scrap off these microorganisms, doe that mean it may not be curable?  I do feel ok when i stsy away from ejaculation for a week or so, but so as i do it gets irritated. Ill let u know what the doctor says, thanks!

You are probably not going to find a lot there. I m sure your bladder is just fine but the doctor might be able to see the urethral passage and see if it looks normal. The procedure wont affect you in any way. A woman's vagina is normally hosting a very long array of microorganisms like anaerobic bacteria, staphylococci, streptococci, various fungi, chlamydia etc. And these cannot be scrapped off with antibiotics. When a vaginal swab culture comes negative it means that the microbiologist hasn't found anything unusual going on. You might be sensitive to some of these bacteria and getting reinfected. An other possibility is that after initial infection your mucus membrane in the urethral passage has a hard time healing its self after recurrent irritation and infection forming scar tissue that its easily injured during sex. It could be a urethritis from mechanical pressure only and you might need to abstain from sex for some time. All these are possibilities. Try to use condom for some time to see what happens. Doxycycline should clear off any non gonococcal urethritis. Let me know what the outcome was.

Yes i have a wife and we have been monogamous for a long time.  I had her go to here pcp and get a culture done. It came back negative. She has no symptoms.  It seems to come back after ejaculation, either masterbation or sex. I think sex is a lttle more rough on the urethr.  I have considered that she could have some type of bacteria but i keep coming up neg, and she also was neg with a culture.  Im going to get a cystoscopy, but my urologist said hes mostly likely not going to find anything, and im afraid that procedure can make matters worse.
