have you tried any other triptans other than Sumatriptan (imitrex)?

because you could also try the following triptans for acute relief:

- Zolmitriptan. brand name is zomig. make sure it is the NASAL SPRAY... NOT THE TABLETS!! tablets will NOT WORK! this is what i use. it does not always work, but i have moderate success with it.
- Rizatriptan. brand name Maxalt. make sure they are the orally disintegrating tablets and NOT THE TABLETS!! they taste AWFUL but will work MUCH BETTER than the tablets. the regular tablets do NOT work very well compared to the orally disintegrating tablets. i have used this in the past and found it very helpful until it stopped working for me. i have friends who use it who swear by it.
- Sumatriptan INJECTION. your doctor will have to teach you how to inject yourself. this is usually only prescribed by neurologists in rare cases of migraines that do not respond to regular triptan treatments. but it works wonders for most people. this is often what they prescribe to those who suffer from cluster headaches as well... which are the most painful headaches that exist pretty much. this is also the type of triptan they give in the hospital ER and Urgent Care Centres.

Do not give up hope on the botox... it can take some time for full effect to take hold. Also, it can be a trial and error thing in terms of your doctor figuring out which trigger points are the right trigger points. Once they have found the right trigger points, you can continue to get botox injections ever 3-8 months as needed or you can even have permanent surgery to severe the muscles where the botox is usually being injected in order for a permanent solution. it may take time, but, if you keep trying with your doctor, you may find that this does turn out to be the answer for you... please don't feel dejected after a week of botox treatment with no results. it can be a long process to find the right tirgger spots... but if you do, you can be rid of your migraines forever.

- How severe is your pain? If you would rate your pain severe or would rate your pain at an 8/10 or higher then have you tried stronger pain killers such as opiates? There are some risks that go along with opiates, the biggest risk of course that most people worry about is dependency and tolerance. Usually in cases where a person is in chronic or severe pain, there does NOT appear to be any scientific or clinical evidence to show that there is a risk of ACTUAL addiction... although, there is STILL a risk of BOTH physical dependency (this is different than an addiction) and a very real risk of developing tolerance to the drug, which means that you need to increase dosages for the drug to be effective (and that increases the risk of dependency). BUT, opiates are VERY effective at killing pain. Depending on the country you live in, you might have difficulty with getting a prescription for opiates. If you live in the United State, chronic pain patients are treated like drug-addicts and life-saving medicines are treated as if they were dirty illicit street drugs. The Controlled Substances Act is an absolutely barbaric law that forces both those in chronic pain and those who are extremely ill (those who have cancer and advanced AIDS) to live in a constant state of torture. It's pretty sick. I thankfully live in Canada. So... if you live in Canada then I would suggest discussing the possibility of treating the worst of your migraines with a LOW and VERY CONTROLLED dose of opiates (I personally suggest Hydromorphone (brand name Dilaudid) over regular Morphine because it is a much cleaner medication with much less risk of side-effects) . I say a LOW and VERY CONTROLLED dose because even though these drugs are NOT addictive if one is ACTUALLY in severe and chronic pain, they are still the sort of things that one can quickly develop a high tolerance and dependency for and any person would really really want to avoid such things... so make sure your doctor understands HOW to prescribe opiates, a pain doctor is the best type of doctor to do this... the risk of dependency and tolerance is dramatically lowered if you have a doctor who knows what they are doing and can switch your medications, lower your doses, etc. as needed. Oh, and that's a good way of avoiding dependency and tolerance, is if you don't stay on the same opiate, or if you go off the opiates and switch to another type of pain killer entirely, for months at a time... and make sure to stop taking the opiates if you are ever out of pain, as otherwise you could THEN develop a serious addiction which could ruin your life just as much as the migraines are currently ruining your life. And always get the smallest prescriptions of opiates as possible, this way you'll never feel at risk of being in possession of something that could be potentially abused by you or anyone else.

Now... if you DO live in America... then, I don't really know... I'd suggest going to the hospital because I know that in America they can give you controlled substances in hospitals if you're admitted and I'm pretty sure just like in Canada, severe headaches are considered pretty serious emergencies. And sometimes all it takes to stop a pain cycle is to get out of pain JUST ONCE. It is possible that one hospital visit could completely stop your pain cycle. If you don't feel like spending the night in an Urgent Care Centre or the ER... then, I would suggest being referred to a Pain Doctor/Pain Specialist instead of a Neurologist. It sounds like at this point, a neurologist isn't going to be the doctor that's going to help you, I really think you need to see a pain specialist. A pain specialist will be able to get you on a proper pain management regime.. which could include strong pain killers like I have mentioned, even if you DO live in America (I am pretty sure this is the only way you can get such medication in the states).

- Even if you wouldn't rate your pain as severe, and you don't think you need very strong pain killers, I still think you NEED to get referred to a pain specialist, they will be able to help you and get you onto the right pain management regime. A neurologist obviously is NOT the answer at this point.

- I saw quite a few daily medications that you have tried. I have tried a few of those as well... amitryptaline didn't work at all for me either ;) don't worry, it's not just you. Actually... I haven't met any migraine patient it's worked for... heh. Anyway... I didn't notice Topamax on that list, that's what I'm on now. You could try that one, it's an anti-seizure drug. Also, I didn't notice lithium on that list, it's an anti-depressant. I'd try the topamax first though. And... there are a whole wack of others to try after those... I can send you an entire list of them if you'd like... from the most common to the really obscure ones.

- But... please don't get dejected!! I know though, I get that way too. I just had a really bad patch of severe migraines for the last month and a half, and only a brief patch of relief between that and a bad patch since Oct... but, I've had a few good days now, so... I have to look to times like right this very second when I do feel dejected. I don't get my hopes up anymore that they'll be gone for good this time or anything, but... I always try my best never to get dejected. Because there is always hope. There is just WAY too much out there I haven't tried yet that has clinical scientific evidence to back it up that has worked for other people... until I try EVERYTHING, I'm not going to let myself actually get analytically dejected. But, I do sympathize and empathize and I'll do whatever I can to help you find the next thing you can try with your doctor. *hugs* stay strong!!

HA! The neurologist prescribed yoga and you are having a chronic 24/7 headache for 10 years?? Gosh... I just love it when doctors pull stuff like that...

Yoga is GREAT... I do physio because I know that my muscles are weak and contribute to my chronic migraines...

BUT! I also have a detailed pain management plan and without it I probably would have killed myself. I still come pretty close to it sometimes!

You are STRONG. Please know that! Most people would have COMPLETELY fallen apart. Even though your headache is taking away a functional life, you have held on to life, and that is more than many in your situation do. Know how strong you are!!!

What you really need at this point is a good pain management routine to help you live a normal life WHILE you try to figure out what is causing this. You aren't anyone's guinea pig. There is no reason for you to be in pain while you or the doctors try to figure out what is wrong. Tell them that you have HAD ENOUGH. Tell them that you need PAIN MANAGEMENT NOW!

There are many forms of acute and chronic pain killers out there. There are pills, patches, injections... I have chronic migraines so I take triptans. Since they have ruled out migraines for you, you probably won't find triptans useful. But, I also take morphine when the pain gets REALLY bad... morphine can stop a really horrible pain cycle. It can take the pain back down to a manageable level... and, when the morphine wears off, the pain stays at that manageable level usually for weeks... sometimes only days or hours... but almost always for weeks... and sometimes even months.

But, morphine isn't the only drug out there. There are lots of weaker and stronger pain killers out there. And... not all are addictive if you are even slightly worried about the possibility of addiction. So... talk to your doctor about all your concerns and they will be able to find a pain-killer that will work for you.

I also get bi-weekly injections. Nerve-blocks to help with the pain. They are great. They are supposed to provide up to a weeks pain relief but I find that they provide one days FULL complete pain relief. BUT... when you live your life 24/7 in pain... that one day of being a completely normal human being is AMAZING and the memory of that can keep you going for a long long time. Although, I know other people who actually get up to three weeks of total pain relief from nerve blocks.

And, I also find a lot of relief in those over-the-counter patches that are for muscle pain (they are like sticky fabric and you can usually find them with all the muscle rub stuff)... I put them on my neck or upper back... since it sounds like you have tension headaches, they might do a LOT for you... it's amazing how well they work! Give them a try... buy a box, it doesn't hurt to try, right?

I have tons more pain management tricks... not all of them have worked for me.. but some of them might work for you... and some of them have provided me with some relief and some of them have been great... give me a shout if you want any more info about anything I've talked about or about any of the other tricks that have failed me but might work for you!

I sympathise with you I have been suffering for about 14/15 years, I know how you feel, trying to lead a normal life is very difficult. I dont know about you but over the years I have tried every remedy known to man and also tried all the above. It amazes me that with all the people in the world suffering with this complaint there is no cure or real help out there. I do hope for all of us there is an answer out there somewhere.

Take care Tiina x

Hi,

You seem to be suffering from chronic daily tension headaches. It will help us to know what medications you have been taking for relief.
I would suggest you to make some changes in your lifestyle like sleep on time, eat regularly, avoid smoking, reduce stress by planning ahead and simplify your schedule. Do some deep breathing exercises everyday and avoid all processed and canned food.

Hope this helps!
