On going migraine

I have suffered from cluster migraine for years BUT this is different.
A week and a half ago I developed severe pain in my left shoulder close to the top. The pain has generated up to the back of my neck and radiates down my left arm. I can't lie down or sit without it causing me grief. I have been taking codeine and paracetamol to get me through but not helping. I will only take it at night to help me sleep but not helping.I have fibromyalgia and post menopause I have sciatica and bulging lower discs.I see an osteopath but unfortunately for this problem not helping.  I'm 59yrs.
I'm at my tether. Please help
