Pelvic adhesions & infertility

Over the course of 16 months I had 4 "natural" miscarriages. Then I had an ectopic pregnancy removed, along with my left tube, 3 weeks ago. When the surgeon inserted the camera she saw that I had pelvic adhesions. My 2 children were delivered normally, I have not had previous abdominal surgery, I don't have endometriosis and I have no previous or recent history of STD's. After each miscarriage they took swabs to check for stds but they have always come back clear. My 2nd child was born 3.5 years ago. I didn't experience any difficulties in conceiving until we started trying for #3.
How have I ended up with these adhesions in my tubes and uterus?
