Why were you sent for an mri?
Is that the exact wording under the "Impressions" or "Conclusion" section? If so, it's definitely too vague for anyone online to be able to assist you. Agree with JJ if the radiologist is meaning that the hyperintensity is on out outside of cord. Normally one would see "focal intensity "in" the cord at C4, or something like that.
You'll definitely have to wait to speak to whichever doctor referred you for an mri to have this clarified.

Your question has come up in the MS community and with MS the lesions develop in the brain and 'within' the spinal cord...from what i understand, your spinal MRI has come up with an abnormal spot at the cervical spine level (C4) but it is 'out side' the spinal cord, so IF it is something of significance and not just an MRI signal glitch (think camera flash) i'm out of my lay persons scope of understanding of what conditions it could be associated with....
Speaking to your doctor will help better explain what these results mean in relation to your medical situation.
