incontinence question

I just wondered how they work out what is neurological and what is not when it comes to the incontinence stuff.  As many know I have the burning in my perineum at the moment, and some bladder issues, mainly urgency, but some hesitancy but of late I have what i call leakage, I DON'T EVEN FEEL LIKE I NEED TO GO TO THE TOILET, and a little bit comes out, enough to make me have to change my pants, so then you race off to the loo to do the rest and your bladder isn't even full, but not enough to be a huge problem, like a pad can catch it?

Is this a MS problem? (neurological) or another problem.  I don't urinate when I sneeze etc. but this happens??? Sometimes I urinate or lose a little bit when I hear things like water (weird) .

My urologist said my problems are neurological/well some of them, but which ones...lol

I am oxybutrin as well, it is the generic of Ditripan, I am also on Avadart too..... the 2 of them together give me horrendous dry mouth.

Same with my Uradynamics... as soon as she started the saline my bladder started spasming and tried to void.... she reduced the flow and it was still trying to void :( The test didn't last long because of this, but she said she had enough information because I failed it miserably!!!

With being only 37 years old, my urgency and incontinence issues are very embarrassing and FRUSTRATING.... Thank God I have an extremely supportive wife and step daughter!

Wow, Scottie, you described me to a T.  When my Urologist did the Urodynamics test they could not get the bladder to fill up.  As fast as they pumped it in I pumped it out.  My doctor put me on Oxybutynin which causes severe dry mouth and skin.

I've been on this medication for a while but it's not really helping.  I also cannot receive the stimulator because of MRI's which I have to have every 6 months.

My last alternative is "The Surgery"  which runs a port out of my side in which I can drain several times a day or else I can hook it to a bag.  Since this doesn't sound very enticing, I go ahead and  wear depends, and take a change of clothes with me.

....and so the life of an MS'er continues on....

I am not a urologist but It sounds like you may have incomplete bladder emptying or urthreral narrowing. I think the first thing to do is get an ultrasound done with a full bladder, then another after they allow you to void and the will confirm or deny incomplete bladder emptying.

Depending on the results the next step should be a scope of your bladder. It will show any narrowing as well as check out you bladder.

If you still are at a loss after those tests, the next test should be a Uradynamics test. They begin by having you void in a funnel type thing and it measures the amount, rate, and flow of your void. The they insert a wire into your rectum, strap wires on your leg, and then catheterize you. They begin filling you bladder with saline and the wires document your bladders reaction throughout the test and can show neurological spasms of the bladder. Once the data is collected they remove the catheter and have you void in the funnel again and the volume will also show if it is incomplete emptying.

I have had all 3 of these test done and didn';t recieve numbing agents or anesthetia and tolerated it well. It is more pressure sensations than it is pain.

I was found to have continual spasms and considering the cost of bladder botox injections it is not an option, the interstim bladder pacemaker is not an option for me because after it is implanted you are unable to have MRI's.

So my conditions are currently being treated with medication, but with limited results. The amount of the 2 medications I take is so high it causes severe dry mouth and dry skin for me.
