Okay.  Let's go with the hypertension thing for a moment.  I hope you did not stop your medicines for that.  But in any case, whomever prescribed those to you, ALSO go back to him, let him investigate and see if you need better medications.  Typically hypertensive people have to make changes to their drugs every few years to stronger and stronger stuff.

This heat or fever thing, it suggests what I've thought all along, which is an infection of some kind, be it in your sinus cavities, or an infection in the brain, which can be something like meningitis.  But I think it's in your sinuses, you've got a raging infection, and it's got to be cleared up.  See, you've had this going on a long time, and just because a doc couldn't find it somewhere back in time, doesn't mean you don't have a problem now.  They could have missed it the first go-around.

I understand completely that you've seen a lot of docs and nobody knows what's going on, and no matter what you do, you have this 24/7 headache.  So, I've given you my very best thoughts on what might be going on, and I suggested what docs to go back to, and things to ask them, and so forth.  I cannot argue my position.  I can only tell you what I think.  If you think it would be no help to go back to some of these docs and get a more intensive workup, then that's your perogative to continue to wonder.  I wish you the best.

Go back to one of the ENT docs, especially since you have sinusitis right now.  And also remember to go back to one of your neurologists.  This headache HAS to be treated.  I mean, there IS a cause of it somewhere, and those are the two specialists that can find it.  And be sure to mention to the neurologist a question about hypertension.

Dear Mr. Alsaihati,
Since your hearing symptoms and occasionally runny nose can suggest a serious sinus cavity infection, which would indeed cause the kind of headache you have, I would suggest you go to an ear nose throat ENT doctor.  When you see him, describe to him as best you can what your awful headache is like, mention your hearing and nose symptoms, and he will know what to do.  He'll take a special scope and look into your nose, your ears, perhaps order a different scan of just your facial cavities, maybe do a hearing test, and it COULD BE that he'll find something going on in your ears and nasal cavities that is giving you this terrible headache.  He would probably use an antiseptic solution to clean out your sinuses, perhaps give you drops for your ears and a spray for your nose, and also give you antibiotic medications, to clear up any infection you may have in your head.  OR he may find an obstruction somewhere in those areas and refer you to a surgeon who can remove it, and that will make your headache stop.

While you are waiting for your appointment with an ENT doctor, you MUST return to one of your neurologists.  You should describe in great detail what the headache is like, tell him you're visiting an ENT physician, and tell him you desperately need some medicines to help you sleep and also to make the headache stop.  Then ask him to perhaps do an MRI scan, sometimes they will pick up what a CT scan will miss.  In addition, you could ask him if there's a chance you have one of the following:  Some sort of meningitis or encephalitis going on (infections in the brain), or possibly a lesion that didn't show up on the previous CT scans, or hypertension and high blood pressure, or a possibility of seizure activity.  Let him decide on various tests he can do, to get to the bottom of this, which would include of course the MRI, also bloodwork to be checked in the lab, and maybe another lengthier EEG (24 or 48 hours, they attach wires to your head and you carry over your shoulder a little halter machine).

In the meantime, I have some home-care suggestions to hopefully help you.  First, keep in mind that lack of sleep can cause a lot of your other symptoms.  So, may I suggest you go to the drugstore and buy some ordinary sleeping pills, like Tylenol PM or whatever the pharmacist suggests.  OR you can go to a health food store and the clerk in there should be able to guide you to some good sleep herbal medicines, like Melatonin or whatever they suggest, a natural ingredient to help you sleep.  Next, perhaps set up a few professional Swedish massage treatments, that can help headaches a lot, and if you find that helps, then also take some very warm tub baths before bedtime, heat helps pain and encourages sleep.  You could also visit an allergist, let him do a little skin test where they determine if you're allergic to some sort of food you eat all the time.  And then there's coffee, if you would drink just a half cup of coffee in the morning, see if it eases up your headache, and if that particular thing helps you, be sure to report it to the neurologist you will be seeing.

In summary, I really think you should go to an ear nose throat ENT physician, I am thinking it just may be some sort of problem with the sinus cavities, which can be connected to your hearing situation, and the whole works can give you an endless headaches (and fevers too).  Also, you must return to one of your neurologists and suggest an MRI, and ask if there's any chance this is a blood pressure problem or brain infection, and see will he give you medicine that is much stronger than what you've been given before, including a new sleeping pill type drug.  While you wait to see these people, get some over-the-counter sleep medicines, maybe get some professional massages, and try a half cup of coffee in the mornings.
