Back pain for last year with left leg involvement

Just needing some advice.  I'm 35 and suffered 2 traumatic injuries involving my lower back.  Most recent MRI says there is a large herniation at L5-S1 which is displacing the S1 nerve root.  My PT says he thinks the root is tethered and that seeing the surgeon is the next step.  I experience weakness in the left leg as well as pain which varies between knee and ankle but is always present in the upper leg.  Also experience low back pain with any movement.  PT has been doing stretching and acupuncture as well as giving me at home exercises, and I am also taking lyrica.  I'm on the fence about surgical intervention but I'm desperate to return to work.  What would be the most successful treatment?  I really don't want to be off work more than I have already and ideally I don't want a reoccurrence.  Thanks for any help!
