Migraines, sharp pain in just one spot, Vision loss, and light headed

Can anyone help me out?
It all started when I was 1, and now I'm 19. I've dealt with headaches and migraines every day it seems like since I was twelve. It's gotten worse. My vision will go completely black when I stand up or sit down. The other night was the first time I ever had a sharp pain like a knife stabbing  me in my left side of my head. It woke me up lasted about 15 minutes and went away. I had a MRI at age 10 due to rapid hearing loss. It came back normal. I have been in and out of the hospital just because I couldn't get rid of a migraine that would last a week. I have no Idea what is wrong.
