Left hand weakness

A 52y lady  with a left hand weakness and inability to grip and control.
She is a diabetic patient with hyperthyroidism, hypertension, arrhythmia and she experienced 2 times blood clots in the brain for short periods.
In 2014 she went to hospital for 2 mouths for DKA and during that time blood clots occurs.
Doctors prescribed heparin as inpatient and warfarin after release,  and she used it for more than 1 year,  it is wrong using it  That long, but she didn't know at that time.
Now she is worry,  wants to know how serious is her situation???
