Toddler's face drooping/pulling and bump on head

My son is 2 1/2 and has fallen and bumped his head over the last year or so repeatedly in the same exact spot on the right side of his forehead.  It has been at least 2 weeks or more since he bumped it last and the bump has still not gone down...it never seems to go away...it is a slight bump but is always there and noticeable.  

So, not sure that is linked to his face "drooping/pulling" but I thought I better mention it.

In the last 3 months my husband and I notice that sometimes when our son is upset, the left side of his face starts to almost pull up toward his eye like a weird half smile.

At first I thought that it was just a facial expression, especially since it seemed to only happen when he was upset, but he was doing it yesterday while watching TV.  It almost seems like it is "stuck" for about 15-20 seconds, but I don't know.

I am trying to decide if these things merit taking him in to a doctor and if so, what type of doctor.  His next yearly checkup is in August.  Plus, since he typically only does it while upset, it would be hard to show it to someone unless we made him upset on purpose.

Please advise.

Without the ability to examine your son and obtain a history, I can not tell you what the best management plan is for him. However I will try to provide you with some useful information.

The bump on your sons head could be a hematoma, or small bruise. However, with multiple episodes of head trauma, particularly since you feel his face pulls or droops, I recommend he be evaluated by a physician. It would be best for you to share your concerns with his regular pediatrician, who may then choose to refer you to a pediatric neurologist. I would take an appointment soon, rather than wait for August.

The biggest concern with repeated head trauma is of course a bleed in the brain though this is very unlikely given that he sounds otherwise fine and this sounds like it has been going on for a while. With the face pulling, one thing that would need to be ruled out is a seizure or other abnormalities of muscle, such as dystonia. Some of these are brought on by stress such as being upset; alternatively, it may be that when his face pulls (for whatever reason), he then becomes upset?

Since his symptoms are episodic, I recommend that you video tape him when his face is pulled so that your physician can see it if he does not have the symptoms while he is at the clinic.
