Chronic means the infarct has been there already for a long time.  If you suffered no stroke-like symptoms in the past, it was likely a 'silent' T.I.A. or mini-stroke that happened and the evidence is showing on the MRI.

Treatment should be aimed at trying to prevent a full-blown stroke from happening in the future.  A neurologist should assist you with this treatment.  Some patients, on the prescription of a doctor, take Plavix (or its generic form), some take aspirin (doctor needs to advise you as to what you should do).

Blood pressure should be kept in normal range so if you have high blood pressure, the doctor will likely want to prescribe medicine to get that down to normal.

Lipid panel (cholesterol and triglycerides) you will want to have at the best possible levels, including your good cholesterol (or HDL) above a certain number- dietary changes are needed if your LDL (bad cholesterol) or triglycerides (which can cause plaque to form in blood vessels where they can get clogged and cause lack of oxygenated to areas of your body, including your brain) are above normal range.  The doctor hopefully will give you a list of foods to avoid, such as saturated fats in large quantities, trans-fats, refined sugar, etc..

If your HDL is too low, increased exercise and, if you can get them, pistachios can help to raise the good cholesterol, which reportedly fights the bad cholesterol.

If you are overweight, you need to get down to a healthy weight and exercise regularly.  If you smoke, you need to quit.
