Help! One says PT, other docs said surgery. Painful shoulder above 90 degrees... What does all this mean?
FINDINGS
BONES: There is no fracture or dislocation. Type II acromion is noted. There is mild acromioclavicular
joint arthrosis with mild narrowing of the supraspinatus outlet.
ROTATOR CUFF: Moderate supraspinatus and infraspinatus tendinopathy is noted. There is partial
articular surface tearing involving approximately 50% of tendon thickness. There is also partial
infraspinatus tendon tearing approximately 1.5 cm from the insertion. The teres minor and
subscapularis tendons are intact.
LONG HEAD BICEPS TENDON: Normal in appearance and location.
LABRAL LIGAMENTOUS STRUCTURES: There is a SLAP tear with extension to the anterior and inferior
labrum and adjacent cartilage .delamination and periosteal tearing. Superior, middle, inferior
glenohumeral ligaments are Intact.
CARTILAGE: There is a rounded area of delamination of the anterior glenoid articular cartilage adjacent
to the labral tear measuring approximately 1.5 x 1.5 cm adjacent to the labral tear.
IMPRESSION
1. Partial supraspinatus and infraspinatus tendinopathy with partial articular surface tearing.
2. SLAP tear with extension .into the anterior and inferior labrum with adjacent periosteal avulsion and rounded area of cartilage delamination measuring 1.5 x 1.5 cm.
3. Type 11 acromion with mild narrowing of the acromioclavicular joint outlet secondary to mild arthrosis.
