Shoulder MRI Report, Help??

I need help understanding my MRI. I have a long history with this shoulder, dating back to 1999, and in 2000 had an impact injury to shoulder and Dr told me that it was permanent damage to the ac joint.

In 2012, I had a torn supraspinatus (partial thickness, 20%) but nothing was ever done to it. In January, my AC joint started bothering me terribly and I couldn't even carry my purse on that side.

Here are the abnormal findings (I will note normal ones at end):

Grade 1-2 glenohumeral joint and superior humeral head articular surface chondromalacia. No acute fracture or distinct osseous sequela of prior dislocation injury

Mild AC joint degenerative arthrosis with a slight laterally downsloping type I acromion. Additional note of AC joint periligamentous edema and periarticular bone marrow edema, likely stress-related and reactive in character

The normal findings:

Intact rotator cuff
Intact and located biceps tendon. Capsule and capsuloligamentous structures maintained
No significant glenohumeral joint effusion, or distinct loose bodies in the synovial space.

I do have autoimmune thyroid disease and its not being controlled, so my immune system has been flaring up. I suspect Secondary Sjogrens and have been tested for RA and ANA panel was done.

Is this something that would likely improve with surgery?
