Identify new pain 1+year post total shoulder replacement surgery?

I had Total Shoulder replacement 2 years ago at age 47, a week post surgery I had an accident, ripped the prosthesis out of socket &  the surgeon went back in and repaired it. I did great, my range of motion and strength is almost 90% of what I had 30 years ago, my problem is a pain I never felt before a pain that starts as a deep bone ache that progresses to a sharp, complete, holy mother of god just cut off my arm now!! type of pain in my entire shoulder, arm and at times my wrist, hand and fingers join in just for fun. The pain is unbearable when I lie down, I no longer sleep more then 2 or 3 -15 to 30 minute intervals a night and the pain can also become worse when I am not actively doing something physical ( It's impossible to sit still, stand still, not move) I don't ask for nor will I take any more narcotic pain medicine as that is it's own misery and not a solution. I'm almost 50 years old and the thought of 20 + years of this pain is unthinkable. I know some will think the pain is all in my head and due to the loss of my old, good insurance, I can no longer afford my previous shoulder surgeon and many specialists don't accept my cruddy insurance. Has anyone ever had this pain? Anyone have suggestions? I do hydro therapy when I can,my daughter is a massage therapist and she helps when she can without over working it even when I BEG her to continue or just to squeeze my arm to stop the pain (who knew too much massage is bad!) I do home PT daily, I don't drink or lift heavy things,nor do I hang from cliffs by my arm( just putting it out there) please...please help.
