Bleeding between period, back pain, please help..

Hi and thank you in advance for your time. I posted last week regarding symptoms I was having that have me scared this might be indicative of ovca. You advised me to follow up with my gyn which I did, but now I am left with more questions than answers. Ill give a brief overview.
I am a 40 yr old woman (obese) with 3 children, (2 pregnancies), took bc pills for only about 2yrs. No hx of ovca or breast ca in family. Have always had regular (normal cycles) every 28-30 days. I episode of midcycle bleed (turned out to be a full period) 3 yrs ago. Dr did tvu and endo biopsy, tvu showed several small cysts, endo biopsy was negative. Since then regular and unremarkable exams.
This month I had typical pms symptoms (breast tenderness and low right back pain) that started a week before my period, but didnt resolve when I started. I saw my nurse practitioner on day 13 who did a cursory pelvic, asked me if it hurt when she was palpating my ovaries, I said no, and she said everything felt fine. The next day (cycle day 14) I had an appt with my GYN, she gave me a breast exam, no pelvic. I explained to her my concern was that it could be a cyst, and she said cysts could cause my symptoms, but wanted to wait and see after my next cycle.
Yesterday, was cycle day 16 and I started to bleed, breast tenderness is all but gone, but the right lower back pain is bad, the bleeding has been very light in two days its only been enough to soak one tampon, it is bright red, not brown. I dont know whether I should consider this a period or break through, I am able to  take advil (2) and feel fine, as soon as it wears off the back pain is back.
I am really hoping for your take on this, as I feel my dr's just arent taking it seriously. Maybe Im taking it too seriously?!  What should I be asking them for at this point?
Unfortunately, I also suffer from anxiety disorder, so for me waiting 3 weeks to start finding out what is wrong is torture. Your help and input is greatly appreciated

I m 32,n three kids i have  3rd child is cissarion ,n during opp they damaged my bllader as  doctor says ,n i was not be able to go home beacuse of  fever then they did DNC ,,n things become bit better  now my 3rd child is 2nhalf yrs old n periods do come at date but fluid is  less like  one n half day bleeding ,n now  i have strated pain in my legs n headces, my gyn doctor says  its normal but i feel something not right,plz advice some tests ,thnks

you have had a really good evaluation.
here are additional things to bring up at your next appointment:
-even with a normal office biopsy of the lining of the uterus (endometrial biopsy), abnormalities can be missed.
The gold standard for the evaluation of abnormal bleeding is a D&C (dilation and curettage).
should you have one to be sure.

Obesity is a risk factor for precancerous and cancerous changes in the lining of the uterus.

Another good test is a pelvic ultrasound. this will evaluate the lining of the uterus to see if it is thick.
