Chemo regime for recurrent OCCC after surgery
As stated before my mother was diagnosed with ovarian clear cell carcinoma with CA125-47.9 in 2006 march after her hysterectomy(no visible mass remained).
She received Cisplatin + Taxol for four cycles and Carboplatin + Taxol for 2 cycles. She was living disease free life upto Dec 11th 2007 when CT scan revealed a small mass on her omentum.
Surgery was done and INFRACOLIC omentectomy was done. (46cmX18CmX2 cm).
No malignancy was seen in her peritoneal cavity. And again No detectable mass remained.
Now her (medicine) doctor is about to treat her with LIPODOX (20mg X 3) + LEDOXAN (1gm) +Palnox (.25gm). I heard that after 18 months of disease free survival CARBOPLATIN + TAxol would be the recommended drug.
As total omentectomy is done(where malignancy was found) and on malignancy is NOT seen in peritoneal cavity would there be a chance of log term remission?
Would LIPODOX (20mg X 3) + LEDOXAN (1gm) +Palnox (.25gm) a good combination as LIPODOX is given for pt resistant recurrence?

Your mother has had good treatment. The drugs she is getting are very reasonable. carbo and taxol are very reasonable to use again if she has another recurrence. It was less than one year from the completion of her first treatment to recurrence. because of this she has platinum resistant cancer. I would not use platinum (cisplatin or carboplatin or oxaloplatin right now. she is at risk for a recurrence again. In general, she may experience that recurrence within the next 12 months. It is common to see a recurrence within a shorter interval from the first cancer free interval. I have also put down a description of ledoxan - cyclophosphamide as others might be less familiar with their use. I am not totally sure what Palnox is. From my search so far it may be palmidronate which is used for osteoporosis. I will check with our pharmacy and see if I can get any further information on this drug.

4. Scalp and body hair may be affected by cyclophosphamide. Your hair may become thinned or brittle and may fall out. This problem may begin about two weeks after the start of treatment, but new growth usually occurs within a few months. The use of scarves, wigs and hairpieces may help you feel better about yourself during this temporary condition.
5. While being treated with cyclophosphamide, a drink plenty of fluids because the drug can irritate your kidneys and bladder. Drink about 10 glasses of fluid and urinate frequently during the first 24 hours after treatment to keep your kidneys working well. This precaution is especially important if you have had chemotherapy before.
6. Let your doctor know if you have difficulty, pain, or bleeding during urination.
7. You should not assume that you cannot get pregnant just because you are on chemotherapy.
