Ovarian cancer vs polycystic ovaries

Can ovarian cancer be mistaken for polycystic ovaries on a transvaginal ultrasound - or do they look different?

They look pretty different, but anything is possible. If further workup desired, MRI may be of benefit.

Ovarian cancer has many symptoms that may be hard to recognise. Because the symptoms of ovarian cancer may be similar to polycystic ovaries, or irritable bowel syndrome (IBS).

TVUS is only one test used to help obtain a diagnosis of ovarian cancer but it, by itself can not be used to diagnose cancer alone. A blood test and other testing including biopsy has to happen to confirm any suspicion of ovarian cancer.
