solid 2 cm ovarian mass- surgery?

HI there- Thanks in advance for your help. I am a 26 y/o female/never been pregnant. For the past 2 years I have had ongoing pain, bloating, nausea and constipation ( this is totally new for me, NEVER before)The only thing ever found pathologically was a 2 cm complex adnexal cyst ( found on CT) Ultrasound confirmed it to be a complex ovarian mass. We have taken a watch and wait approach for 2 years , however this last time it is now a " SOLID NONHYPERECHOIC MASS of 2 cm). My doc referred me to an OB GYN ONC for opinion and he thinks it is most likely endometriosis and i should have a cystectomy and hypsteroscopy with d&C. My symtoms have not changed and although I have daily pain and nausea I " manage" it without medication. While I do have faith in this doctor I would just like someone else's input on my situation. Am I jumping the gun into surgery or would it be ok to watch it a little longer. It seems so TINY to me to be causing all my symptoms?

thanks in advance !

Kris

Hi Krissy,
yes , I agree with your doc. Sometimes a cyst is bigger than the ultrasound suggests

Just wanted to say thanks for your input. As far as dietary change...I am a dietitian ( ironically) and Nothing has helped. I probably eat healthier than the average person..and have had a complete GI workup only to find nothing. Is it possible that this 2 cm growth could actually be larger than it APPEars on ultrasound? My doctor also mentioned that if it was endo it can sometimes attach to the bowel and cause " IBS" type symptoms. I do realize that its Minimally invasive but I just have concerns. My doc also say that if I plan on getting pregnant it could reduce my chance of fertility if it is indeed endo. Thanks again

Dear Kris,
It sounds like you are getting very good care. The surgery that has been recommended is what we call 'minimally invasive' That is, the doctor is going to take a look and get some tissue (from the lining of your uterus and your ovary) for analysis under the microscope.

I agree that is is hard to imagine how a 2 cm cyst could cause all your symptoms. But it could be that you do have endometriosis and the cyst is just a small part of that. It is reasonable to get a diagnosis. There are several good options for managing endometriosis using birth control pills and medicine to induce artificial menopause.
Or it could turn out that you do not have any gyn problems at all. That you have a small fibroma on your ovary and some irritable bowel syndrome problems that should be manged by dietary change. I always think more information is reasonable especially when you have had a chronic problem that is not getting better. That way, you get treated for the right diagnosis.
best wishes to you
