It is not uncommon for a doctor to suggest surgery on a cyst that is 5cm or larger.  Like hopeshell said, cysts that size run the risk of rupturing, plus they can put pressure on your other organs.  Hopefully your cyst has shrunk with the pill and metformin.  Just because they are removing a cyst doesn't necessarily mean that they will remove the ovary.  Usually the ovary has to be removed if the cyst completely surrounds the ovary, if the ovary is stuck to the cyst, or if the ovary isn't functioning.  I would definitely take your doctor's suggestions and talk about possible solutions.  Please keep us updated.

Hi Cathy,

I just saw this post.  I am sorry it wasn't answered before now.  Sometimes a post slips by and doesn't get answered.  I don't know if you have any updates on your cyst.  The reason your doctor probably mentioned surgery is in case your cyst doesn't go away on its own.  Cysts 5cm or larger run a greater risk of rupturing.  If they increase in size then it is necessary to surgically remove them.  It isn't as much about your pain level as it is about the risk of that cyst growing larger and rupturing.  Even benign cysts can cause complications if they rupture.  Hopefully, your meds will be effective at regulating your hormones and you will get some relief from these cysts.  Best wishes!
