I need more info. - can you help?

Hello all- I am hoping someone can help me out here. My daughter is only going on 14, but we made a trip to the ER a couple weeks ago because her school nurse thought she had appendicitis. She had tremendous pain in her right side described as a little over an 8 out of 10. after some blood tests, urine test, and CT scan they found this- an ill-defined mixed density in the high right adnexal region....may represent a complex right ovarian cystic lesion, or inflammatory process. note small amount of free fluid in the cul-de-sac. Since they could not see the ovary well enough to be certain, they suggested an ultrasound- which we did and they couldn't see the right ovary at all, but did see also pelvic free fluid at the upper limits of normal quantity for physiologic fluid. We could not do a vaginal ultrasound because of her age. During the 2 months prior to this episode she had been having gasrointestinal problems such as diarhea, gas, bloating, nausea, etc. I had taken her to the doctor, and also started giving her digestive enzymes and aloe juice- anything to help. She has missed 15+ days of school so far this year. She also gets migraines which are probably not related. But when I took her to a local gyn to follow up the problem she didn't do an exam or follow up with any tests because her pain hasn't been like that since that day. She said that she may have had a cyst that burst when she ovulated and it may not happen again, she said the other symptoms were unrelated most likely. I don't agree- I think the other sypmtoms are definately related- and they should follow up on the test since they never got a good look at her ovary to know what that mass was. The doctor said that they won't see anything now because she's not experiencing the pain now. Do any of you know if this sounds familiar to anything or anyone? I have also considered the possiblity of ovarian cancer. much of the symptoms are the same. Can you all give me more info. about if symptoms come and go with your cycle, or are constant, have gastro. problems played a part in your cysts? are they constant also if so? I didn't know if it can be worse when you ovulate on that side and better sometimes inbetween? can you help?

i believe they did a sonogram (jell on the belly, moving it around). they did see free fluid and that's what caused the swelling and the tenderness on my side. i really don't remember any pain before the rupturing. but once the pain started it lasted for about 2-3 days. after the rupture it was not nearly as bad (more of a swelling, aching pain as opposed to the sharp stabbing pain).

Hi Lynn
As far as i know, it did not rupture, just kept growing to that size.  I had it drained twice vaginally, but it just kept filling back up.  The drained fluid was clear like water and close to 600cc of fluid.  My cyst was a cystedenoma, which is actually a benign tumor on the outside of the ovary.  It did not interfere with my periods, and I had no bleeding.  I did however have a lot of pain and stomach upset during ovulation(lower back pain also, but other side)and it was very sore just before my period.  I had about 3-4 vaginal ultrasounds done and then after the drainage attempts, surgery was scheduled. I had a full laparotomy because of the size and they were able to save my ovary.  The pain would come and go but I had constant pressure on the left side once it got really big.You could especially feel it when you were sitting down.  I had many a headache also, so don't rule that out as something not related. It is very stressful to have things going on with your body and you cannot seem to get answers. As to the doctor saying she needs to be in pain for them to see anything, that is a lie.  Like I said, a lot of the time it was just pressure.  You do need to get her another ultrasound, vaginal is best because it measures size and you can see the type of fluid inside the cyst, but I know her age is a factor in that procedure.  It could be also that if she has a cyst, it is leaking which would account for free fluid in there.  Hope this helps...she is not alone....Ibuprofen works the best for pain and keep pushing for a full pelvic ultrasound!!! Good Luck!!

Thank you for answering- It seems that my daughter's symptoms are a combination of both of yours....I wonder if she had one rupture like lou, but have another large one that caused gastro symptoms like con? or maybe a large one  like yours ruptured - if that's possible? do you know if either of you had free fluid present?

lou486- did you have any symptoms leading up to your cysts rupturing? and do you know what tests they did to allow them to see that was the problem? In my daughters case I feel like they are just guessing. I wonder if she will have it keep happening like you did.

con1974- did your symptoms ever get better, then worse during the month, or from week to week? and also did you ever have pain during ovulation?

i had very similar symptoms when i was a senior in high school. i awoke in the middle of the night with unbearable pain on my right side. i had to crawl all the way down the hall to my parents room. they immediately took me to the hospital and it was suspected that i was having appendicitis, however, after tests, they found that a cyst on my right ovary ruptured causing the agonizing pain.  it happened two more times (the following two months). i then went to the gynecologist where i was put on birth control, and never had a problem since. this may be the best option for your daughter.

hope this helps, and i hope she feels better soon.

I had a 13cm cyst removed from my ovary last year.  I am 34 years old.  I had been complaining about pain in my left side for 2-3 years before they did even an ultrasound.  I also had upset stomach, gas, nausea they said because of the size of the cyst, it was pressing on other organs.  When I first had the symptoms, they told me it was IBS, come to find out 2 years later, those symptoms went away after the cyst was removed.  Keep on it with your gyn, and demand answers, I suffered for 2-3 years with pain they said was nothing...
