I too have PTSD since childhood and it never totally goes away, when we use recovery tools given to us in therapy, the symptoms are managed, but as stress rises and we forget to use our tools, many symptoms sneak back in our lives....You need to tell your therapist what is going on and the 2 of you can discuss either new tools to use or upgrading or increasing the use of the tools you have used in the past to not let PTSD run your life.

I suffer from PTSD from childhood trauma that started when I was 5 until 14. Thanks to a great school Counselor she was able to get the school to pay for outside therapist to come in and help me. I have been able to manage it very well without any medication or episodes except for once back in 03-04 and just recently back in March it hit me again like a ton of bricks.
First, you need to figure out what your triggers are and avoid them when possible. The fear that comes with it will be the hardest thing to overcome and unfortunately as the saying goes to overcome something you have to face it head on. Secondly, find someone who you can talk with, that is the first step in the healing process. The more you talk about it the less you will have an attack. Many find writing in a journal helps, staying active, eating healthy, meditation, breathing techniques, art, something that you have an interest in that is calming and relaxing. Even just screaming out your fears and telling it to go away can be helpful, may seem crazy but it can help.
As far as stress and anxiety it can effect your body in ways you wouldn't think possible and most of the time you won't realize you are until you an episode.
You do need to see your doctor to check for underlining problems with your hormones and I don't know if you have been on any type of contraceptive but some can cause the problems your having, thyroid condition can cause the symptoms your having as well.
Depending on medication if you are taking any they may need to changed until you find what works best for you.

It sounds like a consequence of estrogen dominance ( I read your profile), stemming probably from adrenal dysregulation,
Underlying this is usually unresolved internal and /or external stress, which sometimes is hidden and you may not be aware of.
PTSD, at least partially could be responsible for this.
There are physical, mental and emotional components that need to be
investigated and dealt with, but unfortunately conventional doctors
are not much help in this.
Prescribed drugs are likely to only mask some symptoms, which may help you temporarily, but the possible side effects may not warrant taking them.
My opinion is to find ways to di-stress, even if you are not feeling stressed, like following a healthy diet, staying fit &active, connecting with nature, getting ample rest, avoiding stressful situations, etc.
Have you done any therapy for PTSD and did you get any benefits from it?
Hypnotherapy and Energy Psychotherapy hold a lot of promise in treating
PTSD, often with better & faster results compared to traditional psychotherapy. Look into it.
A Functional Medicine Doctor or ND, knowledgeable in adrenal issues can help you with any Adrenal deficiencies and imbalances, specially when it has to do with sub-clinical conditions, as conventional endocrinologists
do not recognize the adrenal imbalances, even if the symptoms are present.
There are other possible factors and co-factors, but to my opinion, these are the ones to look into first.
