Time For Pain Management?

I sustained severe back injuries in a car accident in 1982. I had a fusion at LL2-5 due to a compression fracture. I also had a hairline fracture between my shoulder blades. I've never been a fan of prescription pain medication and have up until recently only taken over the counter pain relievers. About a month ago, I did ask my doctor for something a little stronger as it seems the older I get, the worse the pain gets. He prescribed Ultram. Problem is, it's not helping. I guess my question is, how do I go about getting into pain management? Or is it even time for pain management? I work 40 hours a week, and have missed two days of work this week because of the pain I am in.

I'm glad you found us at the Pain Management Forum. And I am very sorry you are in so much pain. sandee is correct you pain medication is fairly light. You most likely will benefit from a stronger pain medication. They are a number to select from and you and your physician can discuss which one may be right for you.

It may indeed be time for a PMP. However before I would do that I would discuss your increasing pain with your PCP and determine if he/she is willing or qualified to treat you as a CP sufferer. You may only be experiencing a flare and require short term medication or you may only require a narcotic such as Sandee mentioned once pr twice a day or even periodically. Many times a PCP that have treated you for any length of time are comfortable prescribing a small amount of the less potent narcotics at least for a short period of time.

As many chronic pain Pt's know you can not strive for total pain relief as it is a unrealistic goal. I wanted you to realize this going in although we do get as close as we can to being pain free most do not get to the point where they are totally pain free but close:)

pain management dr saved myn life, i have 3 hernitaed disc and i messed with regular drs. after my back surgery for 11 yrs and was in pain all the time. finally i had a dr that said stop u need a pain management dr and thank god he was smart enough to send me to him.. my pain isnt completly gone but iut is under control....so if u have a chance and can get to one dont walk run if u can

Yes, it sounds like your a candidate for pain management. They can offer you options your PCP can't and you can always decide later to drop out if you feel it is not for you.
As far as your pain meds the ultram is a non narcotic drug that is really not strong and in my opinion if your not getting any relief you need to go to something a little stronger. You are the boss when it comes to what you take but I would suggest asking your Dr about hydrocodone or something similar and see how it does for your pain. You can take it as needed so you won't have to take it on a regular basis unless your pain calls for it. Your in pain and may be for some time so do not be afraid to try the medications that are available to you. You can always opt to stop them if you feel the need or if your pain gets more manageable.
