When I talk to Dr. I loose my cool can you help me?

I always seem to forget what I want to say to the Dr.I have always been this way, my blood pressure goes up, and i always loose my nerve,to say what I want. This month it is very important,I want to ask him to give me the same meds that I was getting for four years prior, I was  doing so well on those meds. I know the worst that can happen is he says no, Can any body give me some advice, so I dont have to find a new Dr.

Try making a list of bullet points you want to discuss. Include why you feel your desire to increase the meds is valid/helpful to you.  
Sometimes making a list of topics/points helps remind me what I want to say
