I am by no means a Methadone expert, but if you are handling the 10mg daily dose fairly well, then most PM docs will probably step you directly up to 15mg per day. Seeing that you have been on 20 before I doubt you are a lightweight and a 5mg increase shouldn't be a big deal for you. Might make you a little sleepy for a few days but shouldn't be too bad overall. As I've said in other posts, a large intake of water every day is very important when you are on medication. Being on 10mg, I'm sure you are probably taking something to prevent constipation, and you may need to increase whatever you're doing if you add 5mg of Methadone to the mix. The water plays an important role in this! Good luck with your increase and keep us updated!

On the other hand, if your doc wants to take it slow and have you break the tabs in half and do 12.5mg's for a week or so and then up to 15, there's nothing wrong with that approach either. You can never be too cautious when it comes to this stuff. Be sure to follow your doc's advice either way.

The safest way to increase the methadone is to request an increase from your physician. Forgive me for not remembering off the top of my head but if I recall you take Methadone for Chronic Pain.
And as CP patients we all know that we never mess with or change a dose unless it is done through our PMP. You risk so much by not doing it correctly. So please hang in there until you are able to discuss this with your physician.
