There is a government website that helps locate Drs. who can do this for you. I'm not sure what is allowed her but if you google "Physicians who prescribe Suboxone or Subutex" you will find the samhsa.gov website.
I'm concerned that you say you have somehow tried the Subutex/Suboxone but don't know of a Dr. to prescribe this to you. This means that you got this from someone else?
A Neurologist or PM Dr. cannot do any type of surgery. The best Dr. to see for spine issues is a Board Certified Neurosurgeon or Board Certified Orthopedic surgeon. Make sure they are Fellowship trained as well.
They can order a new MRI to see if there are any changes from your last one due to the accident. Then they can decide whether to treat you surgically or non-surgically and can use many non opiate means to help along with a Dr. you find to prescribe the Suboxone or Subutex.
