Sub aoirtic VSD

Hello Sir,

My sister's baby boy is born with a sub aoirtic VSD measuring 5 mm with a left to right shunt > 2:1. 2d echo findings are

moderate TR/moderate PAH,
Good LV/RV function,
Intact IAS
No PDA

Can you please let us know if it requires surgical intervention?

We are meeting a pediatric cardiologist also.

Thank you so much doctor. We would monitor the baby. The baby is having breathing problem only during feeding and all other times no issues. Since birth the baby has gained around 0.5 kilo grams and now weighs around 3.7 kilo grams. Age as of today is 28 days.

A 5 mm subaortic ventricular septal defect (VSD) would be considered a moderate sized hole.  This type of VSD does have the capability to get smaller with time as the baby grows. The echo report suggests that the VSD is large enough to raise the pressure in the lungs (moderate pulmonary hypertension) due to a significant amount of blood flow through the hole (shunt > 2:1).  The decision as to whether or not this hole will require surgical repair cannot be answered at this time.  If the baby does not gain weight well, or breathes fast, or continues to have high pressure in the lungs and an enlarged heart during the first 6-12 months of life, then surgery would likely be recommended.  However if the baby grows well, and the hole gets smaller and the pressure in the lungs returns to normal, then it may not need any intervention.  Careful follow up with the cardiologist is recommended.
