vsd with trivial AR

my son is 11 month old and has perimembranous small restrictive vsd ,i did follow up echo for him and i found that the condition is complicated by aortic vale prolapse and trivial AR , so what should i do wiat and follow up or go to surgery ,echo shows tissue growth from right ventricle .

Dear Haad,

Without evaluating your son, I cannot say exactly what you should do.  Certainly perimembranous VSDs are at risk for aortic valve prolapse and valvular insufficiency.  Considering that the leak is trivial, this can probably be observed at the present time.  If there is also tissue growth from the right ventricle, then it is possible that the defect will close from there and not further involve the aortic valve.  It may not be worthwhile to push forward to close the VSD unless you get at least moderate aortic regurgitation.
