Difference in u/s and LMP based dates

I'm 38 years old.  This is my second pregnancy.  I delivered a healthy baby in the first.  According to my LMP, on January 22nd, I should have been 8w 6d.  I had a vaginal ultrasound done and the doctor was able to see a fetal pole measuring 5w 4d.  There was no fetal heartbeat visible. The sac margins appeared crenulated according to her.  She recommended checking my beta HCG levels and a follow-up u/s.  My HCG levels were 27900 on 22nd Jan and only 28600 on 24 Jan.  I did a follow up u/s on 31st Jan.  There were 2 sacs visible, one was shrunk and the other was looking good.  The doctor could not detect a fetal pole or heartbeat.  The sac measure was corresponding to 6w 2d.  During the u/s, I started spotting (dark brown).  The spotting continues.  The doctor said that the pregnancy does not look good, but wanted me to wait and have another u/s after a week.  My periods are fairly regular, with cycles of around 33-35 days.  I would like to understand what the chances are for my pregnancy to me healthy.  If it does result in a miscarriage, are there specific reasons why it happens?

I am so sorry you are going through this...fisrt your numbers coudl be less because you coudl be losing one twin and the other coudl be fine. Next, if you m/c there are sometimes specific reasons that testing could find out but if you had a normal pregnancy before you probably have no specific cause. Unfortunatelyt, m/c can happen fopr a 100 different reasons and most of the time we never know why. Keep follwoing up with HCg and another u/s don't lose Hope yet...the spotting can also be from many things including an infection. You spot brown with BV, bacterial vaginosis, that coudl be triggered by u/s. Again, the best you can do is be positive until you know 100% not to be... each pregnancy is a mysterious miracle and i have heard too many stories to try and even understand one...Hang in there...
