true labor vs false labor

I am 38weeks and have been having contractions since I was 34 weeks. Granted they are strong braxton hicks and they have d ialated me up to 3cm. my mucus plug came out a week ago. This may be tmi but ive also been having multiple bowel movements a day.Everything points to me going into labor any second now but its just not happening. The contractions get stronger every day but they come and go. I feel like when I actually go into labor I won't know (unless my water breaks). I need advice on how u know the contractions are real and when you need to go to the hospital.

My contractions are 5 mins apart and I'm only 1cm dilated. I'm at l&d now. Its not considered active labor until your 4-6 cm dilated. And these things hurt like h e l l

By timing them. If they get stronger and closer together. My doctor told me if I have them 5-7min apart within an hour.
