Thanks I will definitely look into it. I didn't do a birth plan with my last pregnancy and have read about on a few different forums and it's gotten me pretty interested.
I know I want delayed cord clamping this time
No bottles or pacis
No epidural (I didn't want it the first time but because of preeclamsia taking my blood pressure sky high I had to)
I want to latch my baby ASAP to ensure a better nursing relationship from the beginning
So far that's all I can think of

the bump's does have an amazing template. It is super long though. I started with the one on the bump and then found a shorter one and just added in the super important stuff that wasn't listed on the shorter one. I figured that my nurses wouldn't be able to read 7 pages but they might be willing to read 2 in order to get my wants acrossed. Also, check with your birthing center on what their standard procedures are. I found out after the fact that mine requires an IV when you are in the hospital, no choice in the matter. The other part is while you are making a birth plan, remember that things don't always go exactly how you'd like. Everything that could go differently with mine did. By the time I was done, my experience started out as my birth plan, but ended up different due to complications. While pushing, my contractions started getting farther apart, so had to have pitocin which wasn't in my birthplan, and ended up with a c section after everything. So word of advice...roll with the complications. It will save you stress.
The bump has an amazing template just print it and fill it in.
You can look up birth plans on the internet, and sometimes your doctor will provide you with a worksheet for one. Mine gave me a seven page packet for a birth plan that explains what my options for medication/freedom of movement/relaxation tecniques are. It gives me space to write what my feelings are about each option, and covers a lot more then what I wrote above.
Just remember your birthplan is going to be a guideline for them to follow your wishes. It's not a contract for you or them. You can change your mind whenever you would like and if they feel that more intervention is needed for your health or the health of your baby they will let you know.

Basically I have always followed the keep it simple rule when it comes to wording. I try to avoid confusion about anything. As to what it should consist of, you can put anything you want everyone to be aware of but, generally it covers your personal wishes during labor and delivery. You can find a lot of awesome templates and fill in the blank ones on line. :)
