hospitalization????! can anyone help????

I am due dec. 25 by ultrasound. dec 24 by lmp. which puts me at 11 wks 3 days or 11+4 . first dr appt was yesterday. i tested 1+ for albumin in urine. had to do a glucose test, and the dr wants me to do a 24 hour urine catch before my next appt on july 2nd. My first child i had pre eclampsia. i have been having kidney problems since last august. blood in my urine trouble urinating etc.

everything i have read about albumin in urine says that it comes with increased risk for pre term labor, miscarriage and increased risk of pre eclampsia as well as it usually means decreased kidney function. If i do the 24 hour catch and my test results come back abnormal and i do in fact have decreased kidney function does this mean i have a high probability of being hospitalized?
