Unless there is some sort of medical concern, there's no need for one.
I just had my last one at
38 weeks to check the size of the baby. I don't know if all dr.s are the same though
You might if you requested one maybe, but usually after your 20week scan to find out the sex the rest is done by midwife at antenatal appointments with fetal dopplers and feeling, measuring your belly.
Not sure either . My appointment is on the 30th , just ask when you go
I think it depends on your insurance and how many they cover.. i would call them and ask to make sure
