second time around

So I am 31 weeks and for the longest time I have felt pressure down in that region (feels like the baby will pop out and it hurts really bad) lately it has gotten worse and it used to occur only when sitting or laying with my legs level (laying on the bed or couch) it just really hurts! I told my doctor and he says it's normal, but it doesn't feel like it's normal. Anybody else have this same issue? I just worry something might be wrong with how painful it is (sharp quick pains when I move my legs). Also how do doctors determine how big the baby is? With my first I went to a midwife and she said when the baby comes out we will know.  I know they can't be exact but a guesstimate would be nice. (Do you get another ultrasound? )
