Opinions needed please

I went to my obgyn this morning and the doctor always hands me a paper to give to the lady at the desk to checkout and make the next apptment. Well she said this will have to be done at the hospital. I asked what it was for and she said it was a test that came back negitive and I need a shot. Also, the next appointment will be the sugar test that you drink. She said I should of remembered doing all that in my 1st pregnancy. The sugar test I did but never had a shot with my 1st. So she starts looking in my files and says ok nevermind you dont have to get the shot so you just come back to the office next month. The shot she was referring to was the rh one im assumimg. The doc always says test results were good and everything is great. This has me worried that my doctor mixed my files up with someone else. Wrote down the wrong thing. And every other concern there is to think of. If she didnt recheck my file could the shot hurt the baby? Or did the rh test come back negitive and the doc just has it in notes on his personal computer. Also, at 16 weeks they rechecked all blood work and when I asked why. nurse said it was lost or something. The doctor came in and said they just had to retest something and never explained. So now for the opinions.... I have called to switch obgyns' and the down side is I love the hospital I delivered at before and want to deliver this one there too. The chance of my old obgyn being on call when I delivery is low but still a chance. Should I swith and take the chance of him being the doc on call or if he did something out of spite.  Or forgive them of this mistake and hope it doesnt happen again?

It could of been an off day. Honest and nothing else if you haven't had an issue other then that I might brush it off. Lost blood work could be fault of the nurse taking it, not the dr. If your overly concerned switch but it really might be nothing

I would switch. They Seem To Not Have Their Stuff together.

If your not comfortable definitely switch or at least find a midwife at the hospital to feel comfortable with
