We were told to use apple juice for constipation with our first son. Maybe her doctor told her to try it?

At this point,  the baby needs to be getting ALL his calories from breast milk or formula,  unless as maw said the baby is constipated and the doc recommended apple juice.

Formula and breast milk are brain food.  Commercial apple juice is mostly sugar - and getting lots of sugar calories leaves the brain needing more brain food.

I don't think a doctor would ask you to add something sugary to the infant formula. But if it's recommended by her doctor...then ok.

If she's doing it because she thinks formula is not as sweet as breast milk, tell her to try a different brand. Each brand tastes very different. I tasted Gerber, Enfamil, and Similac... and like Similac best. It was the sweetest of the three but not as sweet as breast milk.

If she's doing it to add calories, she doesn't have to do it that way. Just add a little more formula powder to same amount of water. I also added baby rice cereal to the formula on occassion.

Usually you  just use water. If their doctor says it is ok, it's probably fine
