Bleeding at 9 weeks pregnant

Hi again Dr. Brown,

I have posted questions to you on a few occasions now.  Just to give you the history again...I am 40 and pregnant for the 6th time.  All previous pregnancies ended in first trimester miscarriage without ever detecting a heart beat.  I am on prometrium, baby asprin, pre natal vitamens, iron and fish oil. All has been well with this pregnancy up until Sunday.  I started to bleed, and it was very bright red.  It came on with no warning, was like a bubble popped or something. Stopped quickly, then just some light spotting.  I had an ultrasound monday, which showed baby measuring perfectly with a strong fetal heartbeat of 175 (at 9 weeks, 2 days).  He or she was moving around and all seemed fine.  I obviously felt very reassured.  Tech could detect no reason for the bleeding.  Monday I had nothing until the evening, when I had some brown spotting.  Today it happened again in the morning, nothing all day, then again tonight.  Brown and varying degrees of heaviness, although it only shows up when I wipe.  My OB is not interested in seeing me until July 21st as they say we have to wait and see, all looks good and there is nothing they can do anyway.  I found that a bit cold, but she is very good, so I guess I can live with it.  No one has examined me to see if my cervix is closed.  I am a bit of a wreck.  Any thoughts on what could be causing this?  The ultrasound was abdominal.....would it have picked it up if my cervix was partially open?  If my cervix was partially open, would the baby have shown to be doing so well yesterday?   Thanks again, your answers always help so much.

If you were my patient we would probably be ultrasounding you every day with your history!  I tend to do a lot of over kill though for reassurance.  My best guess is that you have a subchorionic hemorrhage which is a separation between the gestational sac and the uterine lining.  This is like a big bruise.  Sometimes they can see it with a transvaginal ultrasound.  Sometimes they can't. Very often there is bright red bleeding initially which is then followed by a brownish discharge as the old blood drains.  If there is no more bright red bleeding, everything will probably be OK.  The subchorionic hemorrhage will heal and the pregnancy will continue.  If you have any more bright red bleeding, this may represent a new bleed, and you should definitely notify your MD.
Often when someone is having this sort of bleeding we will put them on bed rest and pelvic rest (nothing in the vagina).  There is no good evidence that this helps the situation, but it is something to do.  That is really all we have to offer--you are correct that there is nothing else to do anyway.
