Don't eat before you go to sleep, but if you do, eat a salad or just munch on some vegetal afterwards.

I was suffering with the same thing to the point that I was throwing up so much from it I was actually losing weight. My doctor finally prescribed Prevacid, which has worked wonders!!

Milk is def. the best way to go!! If a glass doesn't work, try drinking it through a straw, that always for some reason works best for me!

Sleep or  sit up right for a little and drink some milk. Works for me. I seem to only get it when I drink my favorite juice. MINUTE MAID, yet I cant seem to quit drinking it! lol

Tobbey is right with the milk. I normally only get acid reflux while I am asleep and its bad enough to wake me up. One glass of milk always does the trick for me.

Try sleeping slightly elevated as well - I work with a woman who, when pregnant with one of her kids, actually had to sleep in a recliner for awhile just to keep the acid reflux at bay.  Your doctor could also prescribe something, but I'm not sure if, since you're in the home stretch, you might be able to just put up with it for the next couple of weeks.

Mylanta and rolaids are safe too. I use mylanta seems to work better than tums or rolaids for me. Try not eating for 2 hours before laying down. Don't lay flat either. I suffer from gurd Since I was 15 which has been flaring back up since I became pregnant. There are alot of foods you should avoid also. Anything fried in grease makes it worse.
