Hospital bag

I know people have asked this before, but I need a refresher. What all should I pack in my hospital bag for delivery and when in my pregnancy should I have it ready? I'm 30 weeks now

First baby, I have no idea what to expect, what's given at the hospital, what's not, what's handy for me to have for myself and my fiancé, etc
