Sometimes you just have to take them to their pediatrician. I've Had to do it and they prescribed a cream four my son. But first try letting it dry out then cream or powder
Take the diaper off so the rash can breath. Maybe try a different cream wipes or diapers. And keep it clean.
it could also be the cream you use. desitin burns my daughters but so iuse A&D diaper rash cream. &also keep her diaper off for uh lil that will also help.

This might sound crazy but I have 4 other siblings and everytime we would get any type of diaper rash or basically any pee rash my mother would pour some corn stratch into our diapers and it would immediately been to cool/sooth and heal . It's a home remidy that my mother and grandmother have always used and it's always worked . Plus it couldn't hurt to try . Sounds ridiculous but just trust me on this lol .
Frequent diaper changes to keep it dry will help a lot.

My 2year old had a bad diaper rash due to diarrhea and I tried everything. Milk of magnesia worked great. I would pour some in a container soak the cotton balls in the milk of magnesia and rubbed on the rash I also used the burping cloths with warm water to wipe him instead of the baby wipes since it irritates the rash. My son would get them A lot I would buy him huggies but I change him to brand pamper doctor would tell me he had sinsive skin I would take him warm bath it help and would leave his diaper off
If it's a yeast rash you can use Lotrimin cream (an antifungal) and that should help clear it up. Corn starch can actually promote bacterial growth so that's usually not a good idea to use.
