hcg level is 491 is this normal or indication of blighted ovum?

Hello,
Based on my LMP (12/04/10) I should be about 5 weeks 1 day today. I took a blood test on Saturday (15/4) and the HCG level results came back as 491. My doc says its quite low and the first scan we can do will be this coming Saturday.
This is my 2nd pregnancy, my first was last September which ended in a miscarriage at 9 weeks in November.
I am very anxious and nervous because of the earlier miscarriage, and since doc says it's quite low, can this indicate a blighted ovum?
I had a history of PCOS which I have been adviced to engage in regular exercise which I've been doing for a while. Since the miscarriage, my menses have been quite regular and I average about 30 days between each cycle.
At 491. how far along am I now?

I have pcos and i just found out that i am 4 weeks pregnant. my Hcg levels are 2000. i Had a ultrasound and all they saw was the sac and my doctor told me it is a possible blighted ovum because they didnt see the baby.

Don't know what to do.... i was told last friday i was going to have a MC and i had my levels checked and they are staying at 95..... and my breasts are really really sore and i am getting sick like MS and my ICP is starting...... is it possible i am still pregnant and my body just isn't producing the hormones?? don't know what to do or what to ask my doctor... please help i have gotten my mind Ok for the second time of thinking this pregnancy isn't going to happen and if it is i would like to figure it out... Ya know? any help will be appreciated...

Hi!
If you were my patient, I would repeat the quantitative hcg in 48 hours. A value that doubles over that time period is a good sign. I would also check a progesterone level.
A value above 15 is also a good sign.

There is not too much point in doing an ultrasound until the level of the hcg reaches 2000+ because not much will be visible until that point.

With a level this low, you are either very early, or have a blighted ovum--hopefully the former!

If you were my patient, and if you DID have another blighted ovum, I would discuss Metformin with you.  This medication has been shown in some studies to improve pregnancy rates and reduce the miscarriage risk in PCOS patients.

By the way--continue with your exercise!
