Prostate Cancer

my dad, 59 y/o had nocturia about 3-4 times a night but he drinks water and tea around 12 glasses a day one of which he drinks before he sleeps. no any other symptoms. consulted a urologist had psa. results came at 7.5ug/L he was given xatral. then has psa again results came back with 5.5ug/L was given antibiotic for a month. was ordered to have psa/freepsa. results ar: psa 6.6ug/L ang free psa 1.37ug/L with 21% ratio. he is very worried that it is indeed cancer since it says in his lab result that 24% is suggestive of cancer. he had a history of laryngeal cancer last year and had total laryngectomy so he is very worried. do you think it is really cancer? should he have a biopsy? thanks
