unexplained fever

Hello Dr,
My father 66 years in India is having for the past 3 months unexplained fever between 99-102.5 Dc.All possible tests like Malaria,Typhoid,TB,HIV,Hepatatis B & C , Bone Marrow ,Urinary infection , CSF Lumbar ,Thyroid FNAC,Leptospyrosis ,PCR taken and are Negative.Ultra sound scan of Liver,Chest,abdomen ,Pancreas,neck,Colonoscopy of lower intestine,Rectum and CT scan of abdomen,Head was all normal.He was given lot of antibiotics and paracetamol tabs and injecctions but of no use. 15 days back his Dr.gave NSAID tab (etrocoxib) and the fever stopped.But when he  stopped the NSAID tablet fever has again come.He lost weight 6-7 kgs initially but now he maintains his weight.He has High BP and under went Thyroid operation 8 years back and he takes regular medication for both.He also had Cataract surgery on his left eye 6 months back. Can anyone please guide how to treat him  for possible cure.

The things you mention can be part of a particular chemical exposure that would cause autoimmune issues.  Autoimmune hypothalamus.  Antibiotics are not the answer.  Even high white blood cell counts are a clue to the FATIGUE that usually is also present but not always

Hi,

How is your father? Fever of unknown origin (FUO) was previously defined as a body temperature ≥ 38.3°C rectally for ≥ 3 wk without discovering the cause despite extensive investigation for at least 1 wk. The most common are infections, occult tumors, drugs, and connective tissue disorders. This varies with age, and in your father's case, connective tissue disorders or inflammatory bowel disease are the usual cause.

To reach a definite diagnosis, repeated tests and physical examination needs to be done. Complete and repeated physical examinations, especially of the skin, eyes, nail beds, lymph nodes, heart, and abdomen is important.  Laboratory evaluation generally focuses on detecting infection and connective tissue and neoplastic disorders.To detect connective tissue disorders, serologic tests, such as antinuclear antibody (ANA) and rheumatoid factor, are first obtained as screening tests.

Have they checked his SED rate or ran any tests for any auto immune disorders? My son had fever as a primary symptom and was diagnosed with Familial Mediterranean Fever. You should read about it, very rare and hard to diagnose here in the US but more common in other areas.
