nephropathy needs surgical correction

Randesh (4.5 years), my son had been diagnosed with Acute
Psyelonephrits. This means one of kidneys (right) is being smaller due
to reflux of urine (from bladder to the kidney). It was proven that
the grade of the situation is Grade IV, which is the worse. This was
confirmed through a MCUG scan.

Dr has confirmed that he has to be operated due:

to stop reflux, make the valve at Bladder to one way, if the condition
suits.
If the is dead to remove the kidney.

We have to another scan, DMSA (injecting an ISOtope) before
recognizing which 1 or 2 to be done.

I have no idea where you live, or what hospitals are near you. You must rely on the doctor your son is  seeing you to give you guidance. He will send you to the best facility and surgeon for your son. I wish him the best for this difficult problem.
